Assessment Reports
Doc Regia can turn structured assessment data — collected through a Google Form — into individual reports, one per respondent. This is ideal for performance reviews, student evaluations, inspection checklists, or any scenario where an evaluator fills in a form and the result must be delivered as a formatted document.
The workflow
Step 1 — Create the project
Create a new project in Doc Regia. In the Questions tab, build a form with the assessment criteria you need: the person being evaluated, the evaluator’s name, scores or ratings (using scales, multiple choice, or grids), and a free-text field for comments or recommendations.
In the Template tab, design the report layout. Structure the document with clear sections — for example, a header with the subject’s name and date, a body with scores and observations, and a footer with the evaluator’s details. Use {{placeholders}} to map each form question to its position in the report.
In Folders & DOC, choose PDF if the report should be read-only, or Google Doc if it needs further editing before delivery.
Step 2 — Collect assessments
Share the form link with the evaluators. Each submission generates an individual assessment report automatically via the hourly trigger.
Step 3 — Review and deliver
Open Return Docs to review each report before delivery. You can add notes, correct details, or sign the document. Then return the reports individually or in bulk to the evaluated persons or their managers.
For internal archiving, use Print & Download to export all reports as a ZIP.
💡 Tip: If multiple evaluators assess the same person, create a separate project for each evaluation round. This keeps the reports organised by period and makes year-over-year comparison straightforward.
