Patient Intake Forms

Doc Regia can transform patient intake questionnaires into structured, readable documents — one per patient — ready to be reviewed by the practitioner, added to a case file, or printed for the consultation.

The workflow

Step 1 — Create the project

Create a new project in Doc Regia. In the Questions tab, build a form with the intake fields your practice requires: patient name, date of birth, contact details, medical history, current symptoms, allergies, medications, and consent checkboxes.

In the Template tab, design a structured intake document. Organise the information into clear sections — personal data, medical history, current complaint — using {{placeholders}} for each form field. This creates a consistent format across all patients.

In Folders & DOC, select PDF as the output format if the document should be read-only, or Google Doc if the practitioner may need to add notes after the consultation.

Step 2 — Collect patient information

Share the form link with patients before their appointment — via email, a website, or a tablet in the waiting room. Each submission generates a formatted intake document automatically.

Step 3 — Review

Open Responses to verify incoming submissions, then access the generated documents from the project folder or directly from the Print & Download panel. Print the intake form for the consultation or review it on screen.

⚠️ Important: Google Forms and Google Drive are not certified for storing protected health information (PHI) under regulations such as HIPAA. If your practice handles regulated patient data, verify that your Google Workspace configuration meets the applicable compliance requirements before using this workflow.

💡 Tip: Disable Email to sender for intake forms — patients typically do not need a copy of their own medical questionnaire. Keep Internal email notifications enabled so front-desk staff or the practitioner is alerted when a new intake is submitted.
